One Full-time (100% FTE) Position for Teaching Associate Advanced Practice Provider (APP-- ARNP or PA-C)
Inpatient CCU Service

Faculty Appointment: Teaching Associate/ARNP or PA position in the Division of Cardiology is a full-time, one-year, 12 month appointment, renewable annually (JCC: 0169).

Job Description
The CCU ARNP/PA is a member of the inter-professional Inpatient CCU team that includes attending physicians, cardiology fellows, clinical pharmacologists, physician adjuncts, and nurse practitioners providing care to acutely ill CCU patients. A prosperous applicant will provide CCU care independently and collaboratively with other team members. The position requires a background and education necessary to obtain appropriate clinical privileges, including initial and continuing assessment of the patient's medical, physical, and psychosocial status. University of Washington faculty engage in teaching, research and service.

Reports to: Inpatient Cardiology Service Director and Cardiology Lead Advanced Practice Provider

Minimum Appropriate qualifications:
1. Current and valid licensure as an ARNP or PA-C in Washington State
2. Graduate Degree from an accredited Nurse Practitioner Program or graduate of an accredited PA program, or foreign equivalent.
3. Nationally certified ARNP/PA with DEA
4. Proven excellence working in an interdisciplinary collaborative team environment
5. Excellent interpersonal skills, leadership abilities skills, and clinical skills in the care of patients with chronic and acute cardiovascular illnesses.

Desirable Qualifications
1. Training and practice in inpatient cardiology or CCU/ICU management preferred
2. ACLS Certification

Standards of Practice and Performance
1. Facilitate CCU patient care in consultation and collaboration with attending MD/ fellow, and team pharmacists.
2. Participate in the assessment, planning, implementation, and evaluation of CCU patient care interventions.
3. Collaborate with the attending MD, PCP, and other providers to insure coordination and continuity of care across settings.
4. Attend appropriate clinical meetings to serve as a resource for staff and providers.
5. Facilitate collaboration with other cardiology inpatient and outpatient services to ensure continuity of care.
6. Provide patient and/or family with health education, counseling and referrals to other health professionals and community resources as appropriate.
7. Participate as a key member of the interdisciplinary team through the development of collaborative and innovative practices and evaluation of patient care results.
8. Evaluate quality results for the CCU through participation in the development of quality indicators including:
* Optimizing patient satisfaction
* Appropriate length of stay for complex cardiac patients

CCU Service APP tasks:

1) The CCU APP will provide care for patients on the CCU service. The CCU service will include the following types of patients: Advanced structural heart disease; peripheral extracorporeal life support; acute coronary syndromes; advanced heart failure management including advanced medical therapy; mechanical circulatory support; and transplant evaluations
2) The CCU APP will care for all patients admitted to the CCU service from 8:00am to 8:00pm. The APP will have the primary function of the initial evaluation and writing admission orders.
The primary care of patients on the CCU service will include:
a. Pre-rounding
Receive sign-out from nocturnist by 8:00am
Review overnight events
Perform thorough physical examination
Round with RN caring for patient
Review labs
Review prior day's consults/study notes
Formulate a plan for the day
b. Identify patients to be transferred and begin discharge orders
c. Participate in everyday interdisciplinary rounds with CCU Attending, Cardiology Fellows, Medicine Resident, bedside RN, and pharmacist
d. Provide direct patient care by:
i. Care planning with Attending
ii. Identifying goals for admission
iii. Identifying goal of therapy
iv. Identifying goals for transfer/discharge
v. Facilitating pre-transplant and/or VAD evaluation process
e. everyday order writing, completing everyday progress notes and obtaining consults, communicating with other health care providers.
f. Assume care of patients transferred from other services, ED and transferred from outside facilities.
g. The anticipated census of the CCU Service is up to 17 patients. Care of patients will be shared in a collaborative model between the CCU APP(s) and Cardiology Fellows assigned to the services. As such, each CCU APP will be responsible for caring for approximately 5 patients daily. Division of labor on the CCU service
will follow a daily, alternating, mutually agreed upon pattern, but may be modified, as necessary, located on discussion with the CCU Attending.
h. There will be two CCU APPs on the schedule each day from 8:00am, to 8:00pm, seven days a week. The CCU APPs will work up to an average of 11 shifts per month

3) The CCU APP is expected to maintain excellent interpersonal skills
a) Establish a therapeutic relationship by imparting a commitment to care for the patient and family through the acute illness and beyond
b) Connect with the patient and family by eliciting the patient/family perspective and validating the value of that perspective using strong empathic listening skills
c) Communicate a sense of caring and openness

4) The CCU APP will provide clinical care in the domains of CCU management.
a) Communications with patient/family/provider about disease and treatment and how Information technology relates to patient values, priorities, and goals of care across the spectrum of care.

5) The CCU APP will collaborate with the CCU team and other members of the Outpatient/ Inpatient Cardiac Services.
a) Create patient-specific care plans.
b) Contribute to the plan of care and communicate the plan to the nursing staff, patients, and families
c) Promote a collaborative approach to decision making
d) Ensure appropriate and timely hand-off of patient care to/from members of the team providing daytime and nocturnal coverage of the service.

6) The CCU APP will participate in educating UW School of Medicine students, School of Nursing and PA Program students, and UWMC staff, on all aspects of the CCU care.
7) The CCU APP will participate in the teaching and research mission of the Division of Cardiology by participating in professional and education activities including:
a. Bedside teaching of Medicine Residents and Cardiology Fellows rotating through the CCU service
b. Attend weekly Cardiology Grand Rounds
c. Attend quarterly Cardiology Faculty meetings
d. Collaborate with physician colleagues in identifying patients for clinical trials

8) The CCU APP will assist in program development, developing innovative policies, protocols, and procedures using an evidence-located approach to improve the quality, safety, and value of CCU services
a) Participate with interagency colleagues on CCU Service program development across inpatient, outpatient, and community agencies
